STOMP - Family Health and Fitness Program

STOMP — Solutions & Treatment to Obesity Management and Prevention

STOMP is a fun, interactive childhood healthy weight program that can help your family develop healthier habits.

This six-week program includes workshops on nutrition, fitness, stress management, and more — to help your family grow healthy!

Share your experiences with others and learn how to support your children and family toward healthier living. It’s free for AltaMed patients under age 18 and their families.

Diabetes Group Visits

Diabetes Management Program

Whether you’ve recently been diagnosed, or you’ve had Type 2 diabetes for a while, learning how to successfully manage it can help you lead a long and normal life. This program uses evidence-based principles and focuses on nutrition, activity, medication compliance, and self-care, with the goal of helping you lower your HbA1c scores and keep you in a healthy range.

For six weeks, you’ll meet with a team of professionals in a warm, friendly environment where you can get to know others with diabetes and share your success together. This program is available from the comfort of your home through on-line workshops. The program is available to AltaMed patients over the age of 18.

Diabetes Prevention Program

Diabetes Prevention Program

Join our Diabetes Prevention Program and overcome challenges in developing healthy eating and fitness habits. AltaMed has been recognized by the Centers for Diseases Control (CDC) for the success of this year-long program, and for helping patients reduce their risk of diabetes.  You will receive support from a trained wellness coach, who will help you set and meet your health goals. The program is available to AltaMed patients age 18 and over.

Dietitian Consultations

Dietitian Counseling

No matter your age, what you eat has a huge effect on your overall health. Healthy eating isn’t hard – especially when you have someone to empower you to make good decisions and teach you how healthy food can be delicious. Our registered dietitians provide medical nutrition therapy to members of every age.

Patients with the following medical conditions are encouraged to see a Dietitian: diabetes, heart-related conditions, those considering bariatric surgery, pregnancy, gastrointestinal-related conditions, and patients with any other nutrition-related condition. This program is available to all AltaMed patients at no cost. Ask your doctor for a referral.

Community Health Specialist Program

Community Health Worker Support

Community Health Workers (CHWs) partner with you and your medical team to help you manage your complex health condition(s). CHWs can assist with medication coordination, medical appointments, and access to additional resources. CHWs will meet you where you are and provide hands-on support tailored to your specific needs.

This program requires a provider referral. If you are interested in the program, talk to your provider to find out if you are eligible.

Altamed Pace

AltaMed PACE

AltaMed PACE is the Program of All-Inclusive Care for the Elderly. It was created for seniors who are managing multiple health conditions who need just a little extra help but aren’t ready for assisted living.

With AltaMed PACE, seniors are picked up at their homes and transported in style – you’ve probably seen the vans in your neighborhood! A few days a week, seniors visit a community-focused center where they get medical care and meals and can participate in social activities. Best of all, AltaMed PACE visits and services may be covered by Medicare.

We’re doing whatever it takes to bring good health to our communities — including delivering care and services to where they’re needed most. We provide vaccines and screenings to both AltaMed members and local residents through our mobile units.
